Whether you are an A cup looking to compensate for what you never had, or a saggy B wanting to replace what was lost through pregnancy and breast feeding, you now have a myriad of options. Augmentation mammoplasty (breast enlargement) is usually performed by inserting an implant under the breast either above or below the pectoralis muscle. The resulting scar 2-3 inches long, is generally located in the fold under the breast or half way around the areola. The two hour surgery is performed under light general anesthesia and you are home from the hospital the same day.
All breast implants have a silicone rubber shell which can be smooth or textured (to prevent rotation of the implant). The implants are filled either with saline (salt water) or silicone. Silicone is a cross-linked chain. The longer the chain, the thicker the silicone. Silicone in breast implants now comes in two types. Cohesive silicone is used in round implants. Like Jell-O, it takes the shape of the shell around it. Pressure will deform it but then it returns to its original shape when the pressure subsides. Slice through it and I will not drip or run. Form stable silicone is used in shaped implants (also called tear drop or contour). It is somewhat firmer than cohesive, similar to the consistency of gummi bear candies. It always maintains its shape and like cohesive silicone, when sliced will not drip or run.
Implant sizes range from 100cc to 800cc. Once desired volume increase is determined, selection of the appropriate implant is dependent on dimension- width and height of the chest and desired projection (amount of cleavage). Implants now range from low profile (flat and wide) to high profile (narrow and tall). Shaped implants can be equi-dimension (height to width) or ovoid with one dimension longer than the other.
Smaller augmentations (1/2 to 1 cup) can now be performed without implants by transferring fat from the abdomen, hips and thighs (by liposuction) to the breasts. Enlarging the breasts will minimize breast droop. To eliminate more severe droop, a breast lift can be performed with or without the insertion of implants.
